America is really, truly finished.
So you are absolutely right, but in some markets you have to work for it. In my area it's easier by far to get pricing on a new furnace or a car repair than a medical procedure. You can get pricing. You just have to keep asking until you get someone with a brain. I frequently get, "I don't know the cost, but you may possibly have a co-pay, depending on your insurance." There's a lot of cluelessness.
You left out tax reform in your post of changes needed. A simplier tax system, maybe a flat tax or the FairTax. Either system would do. Everybody gets to pay, not just the working schlub who can't afford high priced accountants and such.
I bet the insurance companies are paying off the critters in DC to not repeal ocare.

afterward, they can consider bringing in new things, such as allowing insurance across state lines, tort reform, etc.
